General Info about these Medical Insurance Claim Forms
CMS(Centers for Medicare & Medicaid Services) forms were formerly known as HCFA(Health Care Financing Administration) forms.
According to CMS, a CMS1500 form (HCFA1500 form) is used by non-institutional providers to bill Medicare Part B covered services and some Medicaid services.
UB-04 forms are generally used for Medicare Part A services.
Some private insurance carriers may also accept claims filed on CMS 1500 (HCFA1500) forms.

Our quote guy for cms (hcfa) forms is really, really old like the bubble mailer guy and he just doesn't do email - but he LOVES the phone! If you call he'll be there at least 95% of the time and if he isn't there he WILL get back with you (unless someone slips up and forgets to give him the message - sorry:) -
Beatin' it up some more:
When you email us, we must call the quote guy (cuz he doesn't do email), get your hcfa or cms quote and then email it back to you, all of which, of course, delays response time. THEN if you have more questions, you'll email us again, then we'll call him again, then we'll email you again...and so on. It could conceivably take a week or so to get a hcfa or cms quote all worked out this way. In reality, if you call him from the git-go it will save you lots of time.
Braggin' a wee bit:
It's usually pretty fun to call us - we don't do voicemail! When you call, a nice person will answer the phone (unless you get that exception and he's the last one in the phone pecking order so your chances of that are slim and most of the time he's even kinda nice).
Yep, our quote guy for hcfa or cms forms is so old he's pre-voicemail! He knows how to use the phone, though, so I guess that makes him post-Alexander Graham Bell - ha!
